Dual chamber permanent pacemaker implantation by femoral approach.

چکیده

Lemos com grande interesse o artigo de Rodrigues et al., intitulado Implantação de pacemaker definitivo (PMD) por via femoral, publicado recentemente na Revista Portuguesa de Cardiologia (novembro/2014), que descreve dois casos de implantação de PMD monocâmara, modo VVI, por via femoral1. No segundo caso descrito o dispositivo foi implantado por bloqueio auriculoventricular (BAV) intermitente de 2.◦ grau Mobitz II e completo (com ritmo sinusal preservado) sintomático. Destacamos ainda o artigo de Valente et al., intitulado Acesso femoral venoso: uma alternativa excecional para a implantação de PMD, publicado na Revista Portuguesa de Cardiologia em maio/2014 onde se descreve um caso de implantação de PMD monocâmara, modo VVI-R, por via femoral em doente com BAV 2:1 (com ritmo sinusal preservado) sintomático2. Embora esta técnica seja conhecida desde a década de 803,4, continua a ser pouco familiar para a maioria dos operadores. Em relação aos casos descritos, gostaríamos de partilhar um dos nossos casos e comentar o tipo de dispositivo implantado. Trata-se de um doente do sexo masculino, 86 anos, com doença renal crónica em programa de hemodiálise com fístula arteriovenosa braquiocefálica esquerda. Foi submetido previamente a implantação de PMD em posição antepeitoral direita por bloqueio auriculoventricular 2:1 intermitente sintomático e posterior explantação de sistema por infeção do dispositivo. Procedeu-se a implantação de PMD dupla câmara por via femoral direita.
